NOTHING LIKE HOME SWEET HOME

Alternative Payment Models Support Patients and Families on Their Path to Recovery

Value-based care models reimburse hospitals, health systems, and other healthcare providers based, in part, on care quality and outcomes. These models allow organizations like ColigoCare, LLC, a clinically integrated network of Valley Health System in Paramus, New Jersey, to coordinate timely care and resources that enable patients to recover safely in their home without needing to return to the hospital.

Arnold's story:

Arnold was on his third hospital stay of the year because of his autoimmune disorder. He wanted nothing more than to go home, but he didn’t know how he could get the care he needed outside of a healthcare facility.

Arnold is a member of ColigoCare’s accountable care organization, which provided enhanced care management services using Medicare’s incentive for value-based care. His ColigoCare team connected him to an outpatient specialist, home health aides, in-home therapy, medication management, and more.

This personalized care helped Arnold transition safely from the hospital back home. He remains out of the hospital, successfully managing his condition and receiving personalized care where he feels safest and most comfortable.

By focusing on patient outcomes and quality of care, value-based care models allow organizations like ColigoCare to help keep their patients health and avoid costly return trips to the hospital. Without continued support for value-based models, patients like Arnold might not get the support they need.
